Application for CPD Providers
A registered person shall have his name entered on the annual list unless he has followed such continuing professional development courses or training programmes dispensed by such institutions or persons as may be approved by the Council.

To be registered as a Speaker, a registered practitioner should fill and submit the following form:

You may apply as Speaker for participating in Continuing Professional Development (CPD) Programme and earn CPD points. Click on the button below to download the corresponding form.
Application for new CPD programme
All registered CPD providers are requested to submit a fresh application for each programme with the relevant details and information.

Registered practitioners can submit the list of approved CPD sessions they attended by filling out this sheet and providing the relevant documents in attachment where applicable. The duly filled form can be sent either by email or by registered post.
